Center Mission

To establish University of Pittsburgh as a national leader in population health management innovation to improve clinical care delivery, promote research, provide educational opportunities and transform healthcare using newer cost-effective models of care.

Goals

  • Develop and implement strategic initiatives for population health management across the Department of Medicine.
  • Synergize Collaborative Partnerships: Foster internal and external collaborations with key stakeholders within and outside the Department and School of Medicine, and UPMC Health Plan and UPMC primary care provider leadership.
  • Increase International and International National Recognition: Increase reputation as a thought leader in population health management
  • Research Initiatives: Lead and support research projects that utilize population health management framework, foster collaborative multi-disciplinary projects for NIH K-, R- and programmatic-level funding, and identify potential external funders such as foundations, industry partners, payors and philanthropic organizations.
  • Clinical Program Development: Support development and implementation of innovative models of care and clinical programs to enhance patient care. 
  • Build next generation of clinicians and scientists with interest in population health: Provide opportunities for trainees and junior faculty to develop research and clinical projects, and provide mentorship and support through Center’s resources.
  • Professional development support: Provide opportunities for professional development skills and networking
  • Biostatistics and Data Analysis support: Provide data analyst support and ensure guidance is available for PHM dashboard development in EHR.
  • Clinical Informatics partnership: Continue partnership with clinical informatics teams to facilitate EHR infrastructure development
