The Center for Population Health Management was established in 2021 with funding support from the University of Pittsburgh Department of Medicine. The Center combines research, informatics and design to build, implement and disseminate new population health management-based models using electronic health record (EHR) that can improve clinical care delivery, promote research and transform healthcare. The Center team can provide guidance for PHM dashboard development in EHR, and operationalizion of protocols in EHR. Center’s Biostatistics Core has developed and refined algorithms for EHR data curation and analysis, which are available to users, alongwith data analyst support. Also, guidance on best practices for protocol implementation, tracking and fidelity monitoring, and expertise with regulatory and compliance processes and best practices for EHR-based PHM is available through the Center.
The University of Pittsburgh is pioneering new strategies to improve outcomes for individual and population health. Partnering with Pitt on an implementation project means collaborating with multidisciplinary teams of clinicians, informaticians, and implementation scientists to accelerate the translation of research into clinical practice.
